staranjem se posamezniki soo\u010dajo z razli\u010dnimi fizi\u010dnimi, psiholo\u0161kimi in socialnimi spremembami, ki lahko bistveno vplivajo na njihovo motivacijo in socialne odnose ter posledi\u010dno na njihovo dobro po\u010dutje (Carstensen, 2021). \u010ceprav je v strokovnih krogih pogosto poudarek na vpra\u0161anjih, povezanih z zdravjem, obstajajo starostne spremembe, ki niso le posredno povezane z boleznimi ali funkcionalnimi omejitvami, ampak imajo enako pomemben vpliv na \u010dustvene, kognitivne in vedenjske vidike kakovostnega \u017eivljenja in spolnosti posameznikov.<\/p><p>V nadaljevanju so obravnavane s starostjo povezane spremembe v motivaciji in dru\u017ebenih odnosih, na podlagi teorij in raziskav s podro\u010dja gerontologije. Z razumevanjem teh sprememb, lahko starej\u0161im bolje pomagate ohranjati zdravo in izpolnjujo\u010do spolnost (Scherrer, 2009).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-97ce13c elementor-widget elementor-widget-heading\" data-id=\"97ce13c\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div dojemanju preostalega \u010dasa \u017eivljenja,<\/span><\/li><li>znali prepoznati spremembe v spolni motivaciji starej\u0161ih, spremenljivost spolne \u017eelje ter pomen intimnosti in novih oblik spolnega izra\u017eanja,<\/li><li>razumeli spremembe v dru\u017ebenih odnosih, kot so spremembe v partnerskih odnosih, vpliv dru\u017ebenega okolja in vlogo socialnih konvojev tekom staranja,<\/li><li>znali ceniti pomen medgeneracijskih odnosov pri podpiranju dobrega po\u010dutja starej\u0161ih in prepre\u010devanju socialne izolacije,<\/li><li>bolje razumeli u\u010dinke kohorte in njihov vpliv na dru\u017ebene in kulturne norme, spolno vzgojo, vlogo spolov in pri\u010dakovanja starej\u0161ih.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<section class=\"elementor-heading-title elementor-size-default\">Motivacijske spremembe v starosti<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b96282c elementor-widget elementor-widget-text-editor\" data-id=\"b96282c\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"font-weight: 400;\">Nekatere teorije staranja pojasnjujejo spremembe vedenja s spremembami motivacije. Cilji, motivacije ter prednostne naloge se spreminjajo skozi celotno \u017eivljenjsko obdobje. Prednostne naloge mlaj\u0161ih se razlikujejo od prednostnih nalog starej\u0161ih.  Za starej\u0161e  na primer reprodukcija ali poklicno napredovanje postaneta manj pomembna, prav tako je manj verjetno, da bodo vlagali v dejavnosti ali odnose, ki jim bodo prinesli dobi\u010dek v daljni prihodnosti. Medtem ko je mlaj\u0161im pomembnej\u0161a \u0161iroka in raznolika socialna mre\u017ea, starej\u0161i bolj cenijo tesne socialne odnose (Carstensen, Isaacowitz in Charles, 1999), kar je povezano predvsem z dojemanjem preostalega \u010dasa \u017eivljenja.<\/span><\/p><p><span style=\"font-weight: 400;\">V skladu s teorijo socialno-\u010dustvene selektivnosti (Carstensen, 1999, 2021) se motivacijske prioritete starej\u0161ih spreminjajo. Ker se njihovo dojemanje preostalega \u017eivljenskega \u010dasa skraj\u0161a, obi\u010dajno dajejo prednost \u010dustveno pomembnim ciljem, kot sta ohranjanje pozitivnih odnosov in izbolj\u0161anje \u010dustvenega blagostanja. Teorija predvideva, da starej\u0161i postanejo izbir\u010dnej\u0161i v svojih socialnih stikih, pri \u010demer se osredoto\u010dajo na kakovost in ne na koli\u010dino.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7e3d00d elementor-widget elementor-widget-heading\" data-id=\"7e3d00d\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Spremembe v spolni motivaciji<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fc8723b elementor-widget elementor-widget-accordion\" data-id=\"fc8723b\" data-element_type=\"widget\" data-widget_type=\"accordion.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-accordion\">\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-2641\" class=\"elementor-tab-title\" data-tab=\"1\" role=\"button\" aria-controls=\"elementor-tab-content-2641\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\"><\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-2641\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"1\" role=\"region\" aria-labelledby=\"elementor-tab-title-2641\"><p><span style=\"font-weight: 400;\">\u010ceprav se spolna \u017eelja med starej\u0161imi lahko razlikuje, raziskave ka\u017eejo, da veliko posameznikov ohrani zdravo zanimanje za spolne aktivnosti vse \u017eivljenje in da je spolna aktivnost \u0161e naprej povezana z u\u017eivanjem \u017eivljenja (Smith et al., 2019). Spolne motivacije se lahko spreminjajo in ka\u017eejo velike razlike med posamezniki. Na primer, spolne dejavnosti, kot so pogosto poljubljanje, bo\u017eanje ali ljubkovanje, prispevajo k bolj\u0161emu po\u010dutju tako pri \u017eenskah kot pri mo\u0161kih, medtem ko spolni odnosi ka\u017eejo ta u\u010dinek le pri mo\u0161kih (Smith et al., 2019).<\/span><\/p><p><span style=\"font-weight: 400;\">V strokovnem kontekstu je \u0161e vedno pomembno priznati, da se lahko spolne \u017eelje in izra\u017eanje med posamezniki, ne glede na starost, zelo razlikujejo in da je treba poskrbeti za vsakega posameznika, ne da bi predpostavljali njegove potrebe in \u017eelje na podlagi starosti ali spola. Splo\u0161no pravilo, ki ga je treba upo\u0161tevati, je: Vse je dovoljeno, vendar ni\u010d ni obvezno. <\/span><span style=\"font-weight: 400;\">Kljub temu lahko s starostjo povezani dejavniki,  kot so telesno zdravje, hormonske spremembe (glej modul 1.2) in dinamika odnosov, vplivajo na spolno motivacijo in izra\u017eanje v poznej\u0161em \u017eivljenskem obdobju. Starej\u0161i v povpre\u010dju poro\u010dajo o zmanj\u0161anju spolnih misli in dejavnosti, vendar njihova potreba po intimnosti ostaja enako pomembna (Kolodziejczak et al., 2019). De Jong Gierveld in sodelavci (2009) so ugotovili, da se je ve\u010dina starej\u0161ih poro\u010denih anketirancev (72 %) strinjala s trditvijo: \"V starosti \u0161e vedno obstaja potreba po ne\u017enosti in intimnosti\". Za mnoge starej\u0161e ima kakovost intimnih dejavnosti ve\u010dji vpliv na zadovoljstvo z \u017eivljenjem kot njihova pogostost (Ska\u0142acka in Gerymski, 2019).<\/span><\/p><p><span style=\"font-weight: 400;\">Podobno so udele\u017eenci ciljnih skupin v projektu EDUSEXAGE (2022) opisali, kako jim s staranjem intimnost postaja vse pomembnej\u0161a. \u010ceprav so priznali dolo\u010deno nostalgijo po aktivnem spolnem \u017eivljenju, ki so ga imeli v mladosti, je ve\u010dina udele\u017eencev bolj cenila in izrazila \u017eeljo po intimnih trenutkih, kot so bo\u017eanje, masa\u017ea, poljubljanje, dr\u017eanje za roke in komunikacija. Ta \"nov\" na\u010din izkazovanja intimnosti slu\u017ei kot dopolnilo ali nadomestek spolne aktivnosti, osredoto\u010dene na spolni odnos. Starej\u0161i so omenili tudi, da s starostjo \u010dutijo svobodo v svoji spolnosti, saj skrbi, povezane z nose\u010dnostjo, vzgojo otrok ali poklicnimi obveznostmi, niso ve\u010d prisotne. Ta na novo pridobljena svoboda jim omogo\u010da, da se povsem osredoto\u010dijo na do\u017eivljanje novih ob\u010dutkov (EDUSEXAGE, 2022).\u00a0<\/span><\/p><p><span style=\"font-weight: 400;\">Medtem ko je du\u0161evni in telesni vir najpomembnej\u0161i za izra\u017eanje spolnosti v poznej\u0161em \u017eivljenjskem obdobju, psihosocialni dejavniki, kot sta spolna zgodovina ali razpolo\u017eljivost partnerjev, predstavljajo ve\u010dino razlik med posamezniki v starosti (Kolodziejczak et al., 2019). Poleg tega je za strokovno prakso pomembno zavedanje, da se posamezniki lahko razlikujejo med seboj tudi po tem, kako \u010dustveno do\u017eivljajo spremembe v spolnosti, povezane s procesom staranja. To ponazarja tudi izsledek preu\u010devanja ciljnih skupin v okviru projekta EDUSEXAGE: Ve\u010dina starej\u0161ih udele\u017eencev je sprejela zmanj\u0161anje libida in sodelovanja v spolnih odnosih kot normalen del staranja. Vendar je ena od udele\u017eenk menopavzo opisala kot travmati\u010dno obdobje, povezano z ob\u010dutki \"staranja\" (EDUSEXAGE, 2022). Vendar pa \u0161tevilni dejavniki postavljajo pod vpra\u0161aj njihovo stabilnost. Prehod v novo obdobje je lahko velik izziv. Mnogi odrasli tvorijo socialne stike znotraj dru\u017einskega ali poklicnega okolja. Upokojitev lahko povzro\u010di ne le drasti\u010dno zmanj\u0161anje \u0161tevila socialnih stikov, ampak, po poro\u010danju ljudi, ki so se upokojili, daje ob\u010dutek nekoristnosti in povzro\u010di upad samospo\u0161tovanja. Ob\u010dutek, da niso ve\u010d privla\u010dni ali spolno privla\u010dni, prav tako vodi k ni\u017eji samopodobi in zaviranju spolnosti. Prepri\u010danje, da  spolnost v starosti ni primerna in da je v krogu prijateljev tabu, lahko prav tako negativno vpliva na posameznikovo spolnost.<\/p><p>S staranjem se pove\u010duje tudi verjetnost spremembe partnerskega statusa ali intimnega razmerja. To je lahko posledica lo\u010ditve ali smrti partnerja. Nekateri ljudje najdejo novega partnerja in odkrijejo nove na\u010dine do\u017eivljanja svoje spolnosti, drugi ostanejo brez intimnega partnerja. Dejansko se odsotnost partnerja najpogosteje navaja kot eden najpogostej\u0161ih razlogov, zakaj se ljudje ne morejo spolno iz\u017eiveti.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<section class=\"elementor-section elementor-inner-section elementor-element elementor-element-2ad37f4 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"2ad37f4\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-inner-column elementor-element elementor-element-fa6a7e4\" data-id=\"fa6a7e4\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-d5d2d8f elementor-widget elementor-widget-heading\" data-id=\"d5d2d8f\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Tisti, ki delajo s starej\u0161imi, lahko pozitivno vplivajo na posameznike, ki se soo\u010dajo s te\u017eavami v  socialnih odnosih, s tem, da jim pomagajo prepoznati in oblikovati pozitivne socialne odnose. Pri tem so jim lahko vodilo naslednji napotki: <\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5eabae0 elementor-widget elementor-widget-text-editor\" data-id=\"5eabae0\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ol><li><strong><em>Teorija socialnega konvoja -<\/em><\/strong>Teorija socialnega konvoja (Antonucci, 1986) predpostavlja, da socialni odnosi tvorijo posameznikov socialni konvoj skozi vso njegovo \u017eivljenje. Pri starej\u0161ih se po navadi sestava socialnega konvoja spremeni, ko do\u017eivljajo prehode, kot so upokojitev, izguba prijateljev ali partnerja in spremembe \u017eivljenjskih razmer. Zdravstveni in socialni delavci se morajo zavedati pomena ohranjanja in prilagajanja dru\u017ebenih odnosov za podporo dobremu po\u010dutju in zdravi spolnosti starej\u0161ih.<\/li><li><strong><em>Medgeneracijski odnosi -<\/em><\/strong>Starej\u0161i redno vzdr\u017eujejo stike z mlaj\u0161imi generacijami, predvsem z dru\u017einskimi \u010dlani in razli\u010dnimi socialnimi mre\u017eami. Ti medgeneracijski odnosi so lahko vir podpore, dru\u017eenja in prilo\u017enosti za dobro sodelovanje starej\u0161ih (Kaufman in Elder, 2003). Zdravstveni in socialni delavci lahko spodbujajo medgeneracijske stike, ter s tem omogo\u010dijo pozitivne socialne odnose in prepre\u010dujejo morebitno socialno izolacijo starej\u0161ih.<\/li><li><strong><em>Dinamika odnosov in komunikacija -<\/em><\/strong>Starej\u0161i se lahko v svojih intimnih odnosih soo\u010dajo s posebnimi izzivi, vklju\u010dno s spremembami v spolnosti in s starostjo povezano negotovostjo. Zdravstveni in socialni delavci imajo lahko klju\u010dno vlogo pri spodbujanju odprte in konstruktivne komunikacije med starej\u0161imi in njihovimi partnerji. Z obravnavanjem njihovih skrbi, zagotavljanjem informacij in nudenjem literature, lahko pomagajo starej\u0161im ohranjati zdrave in zadovoljujo\u010de spolne odnose. Starej\u0161i se o teh vpra\u0161anjih morda la\u017eje pogovarjajo s strokovnjaki s podro\u010dja zdravstvenega in socialnega varstva kot z dru\u017eino ali prijatelji.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-inner-column elementor-element S starostjo povezane fiziolo\u0161ke spremembe ne onemogo\u010dajo ali celo ne ote\u017eujejo zadovoljujo\u010dega spolnega odnosa. Mnogi starej\u0161i pari v svojem spolnem \u017eivljenju najdejo ve\u010dje zadovoljstvo, kot so ga imeli, ko so bili mlaj\u0161i. Morda imajo manj mote\u010dih dejavnikov, ve\u010d \u010dasa in zasebnosti ter nimajo skrbi, da bi zanosili. Prav tako lahko bolje izrazijo svoje \u017eelje in potrebe, kar je lahko prilo\u017enost za ve\u010djo intimnost in povezanost.<\/p><p>Normalno staranje spremljajo telesne spremembe, ki lahko v\u010dasih ovirajo spolnost in u\u017eivanje v spolnih odnosih. Prehod v menopavzo lahko traja ve\u010d let in se kon\u010da, ko \u017eenska 12 mesecev zaporedoma nima menstruacije. Menopavzo lahko spremlja vrsta simptomov. Ti lahko vklju\u010dujejo vro\u010dinske oblive, te\u017eave s spanjem in ohranjanjem spanca ter nihanje razpolo\u017eenja. \u017delja po spolnosti se lahko pove\u010da ali zmanj\u0161a. Spremembe, povezane z menopavzo, ki so posledica zmanj\u0161ane ravni \u017eenskega spolnega hormona estrogena, vklju\u010dujejo zmanj\u0161ano vla\u017eenje no\u017enice, psihoseksualne spremembe, kot so muhavost, razdra\u017eljivost, anorgazmija (nezmo\u017enost dose\u010di orgazem), zmanj\u0161an libido in slab\u0161a spolna zmogljivost (Fritz in Speroff, 2011).<\/p><p>Pogoste spremembe pri starej\u0161ih \u017eenskah, so povezane s spolnimi organi. Pri urogenitalni atrofiji se vagina skraj\u0161a in zo\u017ei, vaginalne stene pa postanejo tanj\u0161e in bolj toge. V ve\u010dini primerov bo vaginalno vla\u017eenje manj\u0161e, vagina pa bo morda potrebovala ve\u010d \u010dasa, da se bo sama naravno vla\u017eila. Zaradi teh sprememb so lahko nekatere vrste spolnih aktivnosti, kot je vaginalna penetracija, bole\u010de in zato manj za\u017eelene. To vse lahko vpliva ne le na spolno aktivnost, temve\u010d tudi na \u010dustveno po\u010dutje, medosebne odnose, telesno podobo in vsakodnevne dejavnosti, kot sta kolesarjenje ali dolgotrajno sedenje (Huang et al., 2010). Hormonsko nadomestno zdravljenje lahko zmanj\u0161a te simptome, pri \u017eenskah pa se lahko ob\u010dutno pove\u010data spolno zanimanje in nagon.<\/p><p>Bole\u010dine pri spolnih aktivnostih lahko povzro\u010dajo tudi te\u017eave z mi\u0161icami medeni\u010dnega dna in tkivi, kot so urinska inkontinenca, cistokela (stena mehurja se pogrezne v no\u017enico), rektokela (stena danke se pogrezne v no\u017enico) ter prolaps\/zdrs no\u017enice ali maternice (zgornji del no\u017enice ali maternice se povesi in pade v no\u017eni\u010dni kanal) (Salonia et al., 2004). Te motnje so \u0161e posebej pogoste pri starej\u0161ih \u017eenskah, pri katerih je zaradi mi\u0161i\u010dnih in \u017eilnih sprememb, povezanih s starostjo in porodom, ve\u010dje tveganje za motnje delovanja mi\u0161ic medeni\u010dnega dna in s tem posledi\u010dno za bole\u010dine pri spolnih odnosih (Walsh in Berman, 2004). Erektilna disfunkcija je izguba sposobnosti doseganja in vzdr\u017eevanja erekcije, pri \u010demer erekcija morda ni tako \u010dvrsta ali mo\u010dna kot neko\u010d (Marais, 2020).\nMed vzroki mo\u0161ke spolne disfunkcije sta najpogostej\u0161a erektilna disfunkcija in hipogonadizem (zni\u017eana raven mo\u0161kih hormonov), katerih pogostost s staranjem nara\u0161\u010da (Kandeel et al., 2001). Fiziolo\u0161ka potenca (sposobnost erekcije, ki je ve\u010dino \u010dasa zadostna za spolni odnos) se s starostjo zmanj\u0161uje. Ena od raziskav je pokazala, da se je v starostni skupini 50-59 let zmanj\u0161ala na 97 %, v starostni skupini 60-69 let na 76 % in v starostni skupini 70-80 let na 51 % (Matthias et al., 1997). Raziskovalci v \u0161tudiji tega zmanj\u0161anja niso mogli pripisati vplivom zdravil ali boleznim, kar nakazuje, da je vzrok spolne disfunkcije lahko le starost.\u00a0<\/p><p>Poleg tega se s staranjem spreminja struktura penisa. Slabe \u017eivljenske razvade kot so kajenje, u\u017eivanje alkohola, pomanjkanje telesne dejavnosti in pogosto gledanje televizije, so povezane z erektilno disfunkcijo (Bacon et al., 2003). Ugotovljeno je, da obstaja mo\u010dna povezava med zadovoljivim spolnim \u017eivljenjem, dobrim zdravjem in visoko kakovostjo \u017eivljenja.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6bccff1 elementor-widget elementor-widget-text-editor\" data-id=\"6bccff1\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong><span style=\"text-decoration: underline;\">Demenca<\/span><\/strong><\/p><p>Pri osebah z dolo\u010denimi oblikami demence se lahko pove\u010da zanimanje za spolnost in telesno bli\u017eino, vendar pa morda ne bodo sposobne presoditi, kak\u0161no je primerno spolno vedenje. Osebe s hudo obliko demence morda ne prepoznajo ve\u010d svojega zakonca ali partnerja, vendar si \u0161e vedno \u017eelijo spolnih stikov. Morda bodo to posku\u0161ali po\u010deti celo z nekom drugim. Raz\u0161irjenost neprimernega spolnega vedenja  med bolniki z demenco se giblje med 2 % in 17 % in je pogostej\u0161a pri mo\u0161kih (Black et al., 2005; Stubbs, 2011). Kako ravnati v tak\u0161nih situacijah je te\u017eko vedeti in utegne povzro\u010diti celo zmedo, saj je razlikovanje med nenormalnim in normalnim spolnim vedenje lahko zahtevno. Neprimerno spolno vedenje lahko povzro\u010di konflikt med spo\u0161tovanjem bolnikove avtonomije ter prepre\u010devanjem psiholo\u0161kih in fizi\u010dnih travm za bolnika in druge (Black et al., 2005; Kamel in Hajjar, 2003). Pred izvajanjem kakr\u0161nih koli strategij obvladovanja neprimernega spolnega vedenja, je treba pridobiti popolno zdravstveno in spolno anamnezo bolnika. Koristno se je izogibati soo\u010denju, saj lahko to povzro\u010di preveliko krivdo ali sram. Hkrati pa neprimernega vedenja ne smemo ignorirati, saj ga lahko s tem nehote okrepimo. Odvra\u010danje pozornosti je lahko za nekatere bolnike zelo koristna tehnika. V domovih za ostarele lahko enoposteljne sobe in zagotavljanje zakonskih ali doma\u010dih obiskov, pomagajo zmanj\u0161ati pogostost tak\u0161nega vedenja, saj zadovoljijo bolnikovo normalno spolno \u017eeljo. Pri bolnikih, ki \u017ee ka\u017eejo znake neprimernega vedenja, je lahko v pomo\u010d izogibanje zunanjim dra\u017eljajem, kot so preve\u010d stimulativni televizijski ali radijski programi. Pri bolnikih, ki so nagnjeni k izpostavljanju ali masturbiranju v javnosti, so lahko v pomo\u010d hla\u010de, ki se odpirajo zadaj ali so brez zadrge. Pri bolnikih, ki \u017eivijo v ne stimulativnem okolju je lahko koristno, da se jim zagotovi ustrezna dru\u017ebena dejavnost. V primeru, da si bolniki napa\u010dno razlagajo spolne navade, je koristno zagotoviti preprosta in ponavljajo\u010da se pojasnila, zakaj je tak\u0161no vedenje nesprejemljivo.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-5983\" class=\"elementor-tab-title\" data-tab=\"3\" role=\"button\" aria-controls=\"elementor-tab-content-5983\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Spreminjanje odnosa dru\u017eine, negovalcev in osebja v domovih za ostarele <\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-5983\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"region\" aria-labelledby=\"elementor-tab-title-5983\"><p>z demenco na domu ali v domu za ostarele zahteva visoko stopnjo tehni\u010dnih in medosebnih spretnosti. Negovalci so pogosto razpeti med moralnimi normami, \u010dlovekovimi pravicami in nudenjem ustrezne oskrbe bolnikom, kar pa lahko vodi v zmedo, jezo, zanikanje, nemo\u010d, v\u010dasih pa tudi v dvoli\u010dnost in apatijo. Ustrezni programi spolne vzgoje za dru\u017eino, negovalce in osebje v domovih za ostarele, lahko izbolj\u0161ajo kakovost nudenja oskrbe in kakovost \u017eivljenja osebam z demenco. Podporna psihoterapija je lahko koristna za zakonce starej\u0161ih oseb, ki ka\u017eejo neprimerno vedenje, saj pogosto potrebujejo zagotovilo, da je tak\u0161no vedenje izraz bolezni in ni odraz njunega odnosa. Eden iz med pokazateljev tega je ugotovitev, da starej\u0161i redkeje uporabljajo kondome v prilo\u017enostnih razmerjih (Holden et al., 2005). Kljub temu pa je po ocenah pri starej\u0161ih zaznana oku\u017eba z aidsom v  9- 11 % primerih (Goodroad, 2003). Raz\u0161irjenost oku\u017ebe z virusom HIV pa se bo pri starajo\u010dem se prebivalstvu verjetno pove\u010devala (Emlet et al., 2019). Starej\u0161i ljudje potrebujejo informacije o varni spolnosti, virusu HIV in drugih spolno prenosljivih boleznih, pri \u010demer imajo pomembno vlogo zdravstveni delavci in negovalci (Emlet et al., 2019). Zdravstveni delavci in zdravstveni sodelavci lahko uporabijo gradivo, ki je na voljo za izobra\u017eevanje o varni spolnosti in je sicer namenjeno mlaj\u0161im, s tem, da ga po potrebi prilagodijo starej\u0161i populaciji.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e805bd3 elementor-widget elementor-widget-heading\" data-id=\"e805bd3\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Starej\u0161i, ki pripadajo spolni manj\u0161ini<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e7e7f75 elementor-widget elementor-widget-text-editor\" data-id=\"e7e7f75\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Osebe, ki se opredeljujejo kot del spolne in spolne manj\u0161ine, kot so lezbijke, geji, biseksualci, transseksualci ali queer, imajo lahko v poznej\u0161em \u017eivljenju dodatne te\u017eave s spolnostjo (Brennan-Ing et al., 2021). Leta skrivanja svoje identitete in drugi dejavniki lahko povzro\u010dijo stres in strah, zaradi katerih imajo lahko starej\u0161i odrasli iz skupine SGM te\u017eave pri odkritih pogovorih z zdravniki ali drugimi izvajalci zdravstvenih storitev.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3dd6b43 elementor-widget elementor-widget-text-editor\" data-id=\"3dd6b43\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>\u0160tudije so pokazale, da:<\/p><ul><li>Starej\u0161i, ki pripadajo spolni manj\u0161ini, imajo v primerjavi s heteroseksualnimi starej\u0161imi ve\u010djo verjetnost, da bodo imeli te\u017eave, ki bi lahko ovirale spolnost in intimnost, kot so ve\u010dja du\u0161evna stiska in ve\u010dja stopnja razli\u010dnih zdravstvenih te\u017eav, kot so bolezni srca in o\u017eilja, debelost in invalidnost.<\/li><li>Starej\u0161i, ki pripadajo spolni manj\u0161ini in \u017eivijo v domovih za ostarele ali drugih okoljih za dolgotrajno oskrbo, so lahko zaradi strahu pred zlorabo, slabim ravnanjem in diskriminacijo manj odprti glede svoje spolne usmerjenosti.<\/li><li>\u0160tevilni starej\u0161i, ki pripadajo spolni manj\u0161ini, svoje spolne usmerjenosti ne razkrijejo svojim zdravstvenim delavcem, tisti, ki so svojo spolno usmerjenost razkrili pa poro\u010dajo o negativnih odzivih (Chan in Silverio, 2021; Beckie et al., 2022).<\/li><\/ul><p>Raziskovalci sodelujejo z ljudmi iz spolnih manj\u0161in, da bi izvedeli ve\u010d o zdravstvenih razlikah in drugih dejavnikih, ki vplivajo na te skupine. S starostjo povezane fiziolo\u0161ke spremembe, kot so hormonske in telesne spremembe, lahko vplivajo na spolno zdravje in zadovoljstvo. Pri \u017eenskah se lahko pojavijo spremembe, povezane z menopavzo, kot so problem z zmanj\u0161anim vaginalnim vla\u017eenjem in psihoseksualne spremembe. Mo\u0161ki se lahko soo\u010dajo z izzivi erektilne disfunkcije in spremembami strukture penisa. Poleg tega lahko razli\u010dne zdravstvene te\u017eave, zdravila in kirur\u0161ki posegi ovirajo spolno delovanje. Vendar pa je pomembno poudariti, da staranje samo po sebi ne onemogo\u010da dobrega spolnega odnosa in da veliko starej\u0161ih v spolnem \u017eivljenju najde zadovoljstvo in intimnost. Za izbolj\u0161anje spolne zmogljivosti v starosti so na voljo razne oblike zdravljenja, kot so nadomestno hormonsko zdravljenje in razli\u010dni na\u010dini zdravljenja spolne disfunkcije. Te izgube lahko vklju\u010dujejo telesne ali psiholo\u0161ke spremembe (kot so pojav gub, razvoj kroni\u010dnih bolezni ali invalidnosti ter upadanje spomina), spremembe osebnostnih lastnosti (kot je ve\u010dja togost), socialne izgube (kot je smrt zakonca ali bli\u017enjih prijateljev) ali vedenjske te\u017enje (kot je ve\u010dja odvisnost od drugih). Po drugi strani pa lahko pridobitve in osebna rast, povezane s staranjem, vklju\u010dujejo ve\u010djo svobodo, potrpe\u017eljivost, dostojanstvo in ob\u010dutek zrelosti ali modrosti (Heckhausen et al., 1989).<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-3362\" class=\"elementor-tab-title\" data-tab=\"2\" role=\"button\" aria-controls=\"elementor-tab-content-3362\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Stereotipi povezani s starostjo<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-3362\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"2\" role=\"region\" aria-labelledby=\"elementor-tab-title-3362\"><p>Ljudje imamo razli\u010dne predstave in pri\u010dakovanja o posameznikih ali skupinah. Ti tako imenovani stereotipi se oblikujejo tekom \u017eivljenja z lastnimi izku\u0161njami, u\u010denjem, z zgledom in znanjem, ki nam je posredovano formalno ali neformalno. Na podlagi skupinskih zna\u010dilnosti posamezniku pripisujemo dolo\u010dene zna\u010dajske lastnosti, vrednote, sposobnosti in pri\u010dakovani videz (Rossow, 2012). Tovrstne predstave so lahko koristne, saj nam pomagajo pri obvladanju socialnih interakcij. Zdravstveni delavci bodo na primer na podlagi predpostavk o ravni razumevanja prilagodili svojo razlago, glede na to ali se pogovarjajo z majhnim otrokom ali odraslo osebo in ali ima oseba medicinsko znanje ali ne. Na ta na\u010din posku\u0161ajo izvajalci zdravstvenih storitev \u010dim bolj izbolj\u0161ati razumevanje in pacientom zagotoviti informacije, ki jih potrebujejo. Vendar pa lahko tak\u0161ne domneve v\u010dasih zavedejo, saj lahko podcenjujejo sposobnosti in znanje sogovornika, zaradi tak\u0161nega komuniciranja pa lahko imajo slednji ob\u010dutek, da jih ne jemljejo resno.<\/p><p>Kako pogledi na staranje vplivajo na delo zdravstvenih in socialnih delavcev, je predmet modula 2.1: Stali\u0161\u010da zdravstvenih in socialnih delavcev do starej\u0161ih oseb, v temi 2: Izzivi in ovire pri odkritem izra\u017eanju spolnosti pri starej\u0161ih.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-3363\" class=\"elementor-tab-title\" data-tab=\"3\" role=\"button\" aria-controls=\"elementor-tab-content-3363\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Samopodoba staranja<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-3363\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"region\" aria-labelledby=\"elementor-tab-title-3363\"><p>Razlikujemo med predstavami, ki jih imamo o skupini starej\u0161ih (stereotipi, povezani s starostjo) in predstavami, ki jih imamo o sebi kot starej\u0161i osebi (samopodoba staranja; Rossow, 2012). Samopodoba staranja se lahko zelo razlikujeje od dru\u017ebenih pri\u010dakovanj ali s starostjo povezanih stereotipov. Raziskave so pokazale, da imajo starej\u0161i pogosto bolj pozitivno samopodobo in pozitivneje dojemajo in sprejemajo lastno staranje, saj se osredoto\u010dajo na svojo osebno modrost, \u017eivljenjske izku\u0161nje in osebno rast (Levy, 2009; Wurm et al., 2017). To je mogo\u010de pripisati izku\u0161nji staranja. Dejavniki, kot so samospo\u0161tovanje, odpornost in socialna podpora, imajo klju\u010dno vlogo pri oblikovanju samopodobe in dojemanju staranja (Hummert, 2011; Sarkisian et al., 2002). Starej\u0161i lahko z ohranjanjem zavedanja identitete, izvajanjem ciljno usmerjenih dejavnosti in aktivnim pristopom k svojemu dobremu po\u010dutju, spodbijajo dru\u017ebene stereotipe (Lamont et al., 2015; Rowe in Kahn, 2015).<\/p><p>Vendar pa vsi posamezniki niso imuni na negativne stereotipe. Kot je bilo poudarjeno, dru\u017ebeni pogledi na staranje pogosto predstavljajo starej\u0161e v negativni lu\u010di in poudarjajo upadanje, \u0161ibkost in odvisnost od drugih (Wurm et al., 2017). Te stereotipe lahko utrjujejo medijske predstavitve, staristi\u010dna stali\u0161\u010da in kulturni predsodki. Sprejemanje in vklju\u010devanje negativnih dru\u017ebenih prepri\u010danj o staranju v lastno samopodobo se imenuje internalizirani (ponotranjen) starizem. Starej\u0161i lahko ponotranjijo negativne podobe in stereotipe, kar vodi do ni\u017eje samopodobe, izgubo samozavesti in odpor do spolnega izra\u017eanja (Levy, 2009).<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-3364\" class=\"elementor-tab-title\" data-tab=\"4\" role=\"button\" aria-controls=\"elementor-tab-content-3364\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Vpliv pogledov na staranje<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-3364\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"4\" role=\"region\" aria-labelledby=\"elementor-tab-title-3364\"><p>Obstajajo razli\u010dne \u0161tudije o tem, kako lahko pogledi na staranje vplivajo na lastne izku\u0161nje s staranjem (Wurm in Huxold, 2012). Ugotovljeno je bilo, da so negativni pogledi na staranje povezani s starostno diskriminacijo ali internaliziranim starizmom disfunkcionalna. Ljudje na primer ne gredo ve\u010d k zdravniku zaradi pri\u010dakovanja neu\u010dinkovitosti obravnave ali zaradi simptomov, ki jih neupravi\u010deno pripisujejo starosti, kar onemogo\u010da pravilno diagnozo (bole\u010dine v hrbtu, \"senilna depresija\"). Pretirane pozitivne predstave o starosti  lahko vodijo v frustracijo in razo\u010daranje, \u010de na primer \u010dlovek ne more izpolniti lastnih pri\u010dakovanj ali \u010de fiziolo\u0161kih sprememb starosti ne more sprejeti  (spremembe na ko\u017ei ali laseh). Zdi se, da se najbolje obnesejo ljudje, ki imajo diferencirano samoreferencialno podobo starosti in ki lahko dopu\u0161\u010dajo dolo\u010deno stopnjo tolerance do individualnih sprememb v obeh smereh.<\/p><p>Tudi pogledi na staranje imajo pomembno vlogo pri spolnosti v starosti (Schroeter, 2012). Nekateri ljudje lahko ob\u010dutijo razliko med svojimi potrebami in \u017eeljami, ki jih \u0161e naprej do\u017eivljajo s staranjem, in tem, kar po njihovem mnenju od njih pri\u010dakujejo ali kar so pri\u010dakovali sami. Negativni prikazi v medijih, kot je odsotnost starej\u0161ih odraslih v intimnih odnosih ali osredoto\u010danje na s starostjo povezan telesni in kognitivni upad, lahko prispevajo k ageisti\u010dnim prepri\u010danjem in vplivajo na samopodobo in samozavest starej\u0161ih posameznikov v zvezi z njihovo spolnostjo.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-3365\" class=\"elementor-tab-title\" data-tab=\"5\" role=\"button\" aria-controls=\"elementor-tab-content-3365\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Opolnomo\u010denje in sprememba diskurza<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-3365\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"5\" role=\"region\" aria-labelledby=\"elementor-tab-title-3365\"><p>Spodbujanje pozitivnih podob staranja lahko okrepi starej\u0161e, saj spodbija stereotipe ter spodbuja bolj vklju\u010dujo\u010do in raznoliko predstavitev spolnosti starej\u0161ih. Izpostavljanje primerov starej\u0161ih, ki so spolno aktivni, sprejemajo svojo spolnost in izpodbijajo staristi\u010dna prepri\u010danja, lahko pozitivno vpliva na samopodobo in vedenje.<\/p><p>Zdravstveni in socialni delavci imajo lahko pomembno vlogo pri spodbujanju pozitivne samopodobe in izbolj\u0161anju spolnega po\u010dutja starej\u0161ih. Negativni stereotipi, starizem in slaba telesna samopodoba in zmogljivost, lahko prispevajo k zmanj\u0161anju samospo\u0161tovanja ter nizki samopodobi in odporu do izra\u017eanja spolnosti. Zdravstveni in socialni delavci lahko te vplive prepre\u010dijo s spodbujanjem pozitivnega pogleda na staranje, spodbijanjem stereotipov in opolnomo\u010denjem starej\u0161ih z izobra\u017eevanjem, podporo in odprto komunikacijo. S spodbujanjem pozitivne samopodobe in z odpravljanjem pomislekov glede pogledov na staranje, lahko strokovnjaki pomagajo starej\u0161im  pri zdravi spolnosti in izbolj\u0161anju njihovega
